The Manitoba FASD Centre operates on a referral basis and currently serves pre-school and school-age children as well as adolescents where there is known prenatal exposure to alcohol.
Referrals can be made by:
- Physicians, Health Care Professionals, Child and Family Services, Schools, and other community professionals.
- These referrals are made by completing the referral form available for download below and sending it to the centre by mail or fax (to maintain confidentiality, the information cannot be sent via email).

- Family members can contact the FASD Centre directly to initiate a request for an assessment. Staff at the Centre are available to meet with parents to complete the referral process.
Once a request for an assessment has been received, the initial process focuses on acquiring accurate information to support the assessment and diagnosis including: family history, birth history, exposure to alcohol during pregnancy,
medical history, developmental history and reports from daycares and schools
regarding development, learning and
behaviour.
Criteria for an assessment include a confirmed history of prenatal exposure to alcohol and developmental, academic or behavourial concerns.
Note: not all referrals will necessarily result in an assessment for FASD.
Here's how the process proceeds:
- The legal guardian is sent an intake package which includes:
information form, school questionnaire, daycare questionnaire, consent to release birth information, and consent for the assessment.
- Staff at the centre are available to families or other professionals to answer questions and assist with completion of the forms.
- Families or referral sources are asked to forward copies of any assessments previously completed at school or in the community.
- The Centre may contact the family to gather more information.
- The Centre will send a letter to the family and source of referral outlining the expected waiting period for the assessment appointments.
